BARANGAY LISTING OF SERVICE FACILITIES AND GOVERNMENT PROJECTS
Dear Sir/Madam,

The Philippine Statistics Authority (PSA) is currently implementing the Community-Based Monitoring System (CBMS), pursuant to Republic Act No. 11315, otherwise known as the CBMS Act. The CBMS, as defined by the law, refers to an organized
technology-based system of collecting, processing and validating necessary disaggregated data that may be used for planning, program implementation and impact monitoring at the local level while empowering communities to participate in the
process.

One preliminary activity to be conducted for the 2024 CBMS, which will be conducted simultaneously with the Census of Population (thereby referred to as 2024 Census of Population and Community-Based Monitoring System, or the 2024 POPCEN-
CBMS) is the administration of the Barangay Profile Questionnaire (POPCEN-CBMS Form 5) and the Barangay Listing of Service Facilities and Government Projects (POPCEN-CBMS Form 6). This undertaking aims to capture and collect relevant and
timely information in the barangay on essential basic services and infrastructure, and to generate a reference list that shall be used in the 2024 POPCEN-CBMS Geotagging of Service Facilities and Government Projects. Meanwhile, geotagging will be
undertaken to gather spatial (location) and attribute (name, sector, implementing/operating and funding institution, etc.) data of these structures in the locality. The collected data will be used in generating thematic maps, serving as effective tools to
come up with sound decisions in making programs and allocating resources for economic development and poverty alleviation.

All barangay LGUs are requested to accomplish this form completely. Meanwhile, by filling out this form, you are authorizing PSA to collect data about your barangay. Such data will also be shared with national government agencies for monitoring
purposes, as well as for providing possible assistance to the LGUs.
