2. This pertains to the Memorandum from EVELYN M. DOMINGUEZ, Ph. D,


Vice
President for Administration to conduct investigation regarding the Complaint Letter of
Mrs. Sylvia Q. De Rueda.

3. Mrs. Sylvia Q. De Rueda is presently assigned at Book Store as Procurement


and Property Custodian located at the back of MMC Canteen.

4. Mrs. Sylvia Q. De Rueda is the holder of key intended for the steel cabinet.

5. Mrs. Sylvia Q. De Rueda pick-up the key of Book Store to the Guard-on-Duty
on November 29, 2023 at 8:34am.

6. Mrs. Sylvia Q. De Rueda is half day duty on November 28, 2023.

7. In her complaint letter, she mentioned on her statement that the collection
money for uniforms was kept inside the steel cabinet locker with padlock on November
25, 2023.

8. At around 11:00 am of November 30, 2023, Mrs. Sylvia Q. De Rueda reported


the incident to the Office of VP for Administration and VP for Finance regarding the loss
of money amounting of One Hundred Forty-One Thousand and Five Hundred Pesos
(Php141,500.00) that was placed inside the steel cabinet for safekeeping.
9. She mention that the steel cabinet has scratched that it may be possible that
somebody destroyed the padlock and took up the said money inside the cabinet.
Apparently, although the steel cabinet have scratched and hard to open (may be due to
it is being used for a long time). The padlock is not deformed or misaligned.

10. Upon physical inspection made by the undersigned together with


complainant, we found out that there is NO forcible entry of intruder both the main
entrance door and back jalousies window as well as the dust and web in between the
glass and aluminum frame are still intact, the window sill is no tracing/marking of
footprints. No disarray of things inside the room.

11. The complainant verbally request to the undersigned to conduct the


actual
review the CCTV footage from 6pm of November 24, 2023 to November 29, 2023 @
6am in the morning except day time. The result during review has no indication that
somebody entered the main door including at back window of the Book Store.

12. Records show that on November 29, 2023 at around 8:34 in the morning,
She
pick-up the key of Book Store Office upon the presence of L/G Lopez (Guard-On-Duty).
Subsequently, she failed to return the key to the Guard on Duty for almost 48hrs. She
performed half day duty only on November 28, 2023. (See attached Tab-D & Tab-E)

13. Ms. Rusby Mahinay is cleared on the alleged loss of money due to absences
on November 25, 28 & 29, 2023 excluding Sunday & Holiday as mentioned by the
complainant.

14. After careful evaluation, It is clearly that the One Hundred Forty-One
thousand and Five Hundred Pesos (Php141,500.00) was loss under the custody of
complainant.

15. In view of the foregoing, this office finds probable cause to indict the
complainant for neglect of duty for failure to remit the said money on time and she is
responsible the safety of the Book Store Office.

16. Recommends Approval of para.14 & 15.
