History
Cape Fear Valley Medical Center was

opened in 1954 as a 200-bed county


hospital. Which is located in the growing
Metropolitan City of Fayetteville, NC. Cape
Fear Valley Medical Center specializes in
heart care, cancer treatment and surgical
services. A family Birth Center as well a
level III Neonatal Intensive Care Unit (NICU)
to care for premature babies are services
that are provided here.

Care/case management is the process of

Consistently managing the quality cost and


delivery of medical services across the care
continuum through disease/Management,
discharge planning utilization management
and Quality Management
Nurses and Medical Social Workers work in
partnership to meet the needs of patients
and their families by establishing safe and
effective discharge planning process.
