 Hepatitis C adalah suatu
penyakit yang disebabkan oleh
virus hepatitis C yang dapat
menimbulkan peradangan
bahkan kerusakan sel –sel hati.

 Virus ini dapat mengakibatkan

infeksi seumur hidup, sirosis
hari, kanker hati dan kematian.

 Belum ada vaksin yang dapat

melindungi terhadap HCV, dan
diperkirakan 3% masyarakat
Indonesia terinfeksi virus ini.

 Penyebab hepatitis C adalah Virus

hepatitis C yang temasuk kelas
Flaviviridae genus hepacivirus dan
memiliki selubung glikoprotein
dengan RNA rantai tunggal.
I. THE EPIDEMIOLOGY
EPIDEMIOLOGI VIRUS HEPATITIS C (HCV)
HEPATITIS C VIRUS (HCV)
Hepatitis virus akut merupakan infeksi sistemik yang
dominan menyerang hati.

Hampir semua kasus hepatitis akut disebabkan oleh

salah satu dari lima jenis virus meliputi virus hepatitis A
(HAV),
virus hepatitis B (HBV), virus hepatitis C (HCV), virus
hepatitis D (HDV) dan virus hepatitis E (HEV).

Semua virus tersebut merupakan virus RNA kecuali virus

hepatitis B.

Umumnya genotip yang ada pada penderita adalah:

Tipe genotip 1, genotip ini lebih sulit untuk ditangani &

membutuhkan waktu pengobatan yang panjang yaitu
1 tahun,
Umumnya terdapat pada orang Indonesia.

Tipe genotip 2, membutuhkan waktu pengobatan relatif

singkat yaitu 6 bulan.
Di Indonesia belum terdapat laporan resmi mengenai
infeksi VHC, namun menurut laporan lembaga tranfusi
darah didapatkan sekitar 2% positif terinfeksi.

Pada studi populasi umum di Jakarta prevalensi VHC

sekitar 4%. Pada umumnya transmisi terbanyak adalah
berhubungan dengan tranfusi darah terutama yang
dilakukan sebelum penapisan donor darah untuk VHC
oleh PMI.

Infeksi VHC juga dihubungkan dengan status ekonomi

yang rendah, pendidikan kurang dan perilaku seksual
yang berisiko tinggi.

Beberapa laporan juga menyebutkan bahwa dapat terjadi

infeksi VHC melalui tindakan-tindakan medic seperti
endoskopi, perawatan gigi, dialysis maupun operasi.

Selain itu, VHC juga dapat ditransmisikan melalui luka

tusukan jarum. Pada umumnya, genotip yang didapatkan
di
Indonesia adalah genotip 1 (sekitar 60-70%) diikuti oleh
genotype 2 dan 3.

Prevalensi yang tinggi didapatkan pada pasien pengguna

narkotika suntik (>80%) dan pasien dialysis (70%).

 Daur Hidup Virus Hepatitis C
HCV masuk ke dalam hepatosit
dengan mengikat suatu reseptor
permukaan sel yang spesifik.
Reseptor ini belum
teridentifikasi secara jelas,
namun protein permukaan CD8
adalah suatu HCV binding
protein yang memainkan
peranan dalam masuknya virus.
Salah satu protein khusus virus
yang dikenal sebagai protein E2
menempel pada reseptor site di
bagian luar hepatosit.
Kemudian protein inti dari virus
menembus dinding sel dengan suatu
proses kimiawi dimana selaput
lemak bergabung dengan dinding sel
dan selanjutnya dinding sel akan
melingkupi dan menelan virus serta
membawanya ke dalam hepatosit.
Di dalam hepatosit, selaput virus
(nukleokapsid) melarut dalam
sitoplasma dan keluarlah RNA
virus (virus uncoating) yang
selanjutnya mengambil alih peran
bagian dari ribosom hepatosit dalam
membuat bahan-bahan untuk
proses reproduksi.
Virus dapat membuat sel hati
memperlakukan RNA virus
seperti miliknya sendiri.
Selama proses ini virus menutup
fungsi normal hepatosit atau
membuat lebih banyak lagi
hepatosit yang terinfeksi
kemudian membajak mekanisme
sintesis protein hepatosit dalam
memproduksi protein yang
dibutuhkannya untuk berfungsi
dan berkembang biak.
RNA virus dipergunakan sebagai
cetakan (template) untuk
memproduksi masal poliprotein
(proses translasi).

Poliprotein dipecah dalam
unit-unit protein yang lebih kecil.
Protein ini ada 2 jenis yaitu
protein struktural dan regulatori.
Protein regulatori memulai
sintesis kopi virus RNA asli.
Sekarang RNA virus mengopi dirinya
sendiri dalam jumlah besar (miliaran
kali) untuk menghasilkan bahan dalam
membentuk virus baru.
Hasil kopi ini adalah bayangan cermin
RNA orisinil dan dinamai RNA negatif.
RNA negatif lalu bertindak sebagai
cetakan (template) untuk memproduksi
serta RNA positif yang sangat banyak
yang merupakan kopi identik materi
genetik virus.
Proses ini berlangsung terus dan
memberikan kesempatan untuk
terjadinya mutasi genetik yang
menghasilkan RNA untuk strain baru
virus dan subtipe virus hepatitis C.
Setiap copy virus baru akan
berinteraksi dengan protein struktural,
yang kemudian akan membentuk
nukleokapsid dan kemudian inti virus
baru. Amplop protein kemudian akan
melapisi inti virus baru.
Virus dewasa kemudian dikeluarkan
dari dalam hepatosit menuju ke
pembuluh darah menembus membran
sel.
 III. DIAGNOSIS
Rangkaian tes yang digunakan umunya adalah tes EIA untuk
mengukur antibodi Hepatitis,tes HCV RNA hepatitis untuk mengukur
level virus & genotip virus, dan biopsi hati untuk mengukur tingkat
keparahan akibat Hepatitis.

Biasanya, tes pertama pasien akan mendapatkan tes skrining yang

akan menunjukkan apakah dia telah mengembangkan antibodi
terhadap virus Hepatitis C atau tidak. (Antibodi adalah suatu zat
terdapat dalam darah yang diproduksi oleh tubuh sebagai respon
terhadap suatu virus).

Jika hasil tes antibodi positif berarti bahwa seseorang telah terkena
virus pada suatu waktu, kemudian dokter kemungkinan besar akan
melakukan tes kedua untuk mengkonfirmasi apakah virus masih ada
dalam aliran darah atau tidak.

Jika terdapat virus maka maka tes genetik akan dilakukan untuk
memeriksa tipe genotip HCV.
