Procedure of Operation
•Prepare the tools and drugs for general anesthesia
•Positioning the patient on the operative bed with supine position
•Premedication through IV line with midazolam (0.07 to 0.1 mg /kg): 0.07 to 0.1 x 70 kg = 4, 9-7 mg

•Analgetic through IV line:
fentanyl (2-150 mcg /kg): 2-150 x 70 kg = 140-10500 mcg (Given mcg)

•Induction through IV line:


propofol (2-2.5 mg /kg): 2-2.5 x 70 kg = 140-1175 mg (Given mg)

•Muscle relaxant through IV line:


atracurium (0.5-0.6 mg /kg): 0.5-0.6 x 70 kg = 35-42 mg(Given 30 mg)

• Oxigenation with a facemask in 3 minutes
• Put the endotracheal Tube number 6, 5, Confirmed the location of the balloon
and ET fixation
• maintenance O2 3 lpm, N2O 3 lpm, and sevoflurane
• Same ventilation with respiratory rate of the patient
• After the operation was finished, clean the airway with suction and give reverse
medication:
• neostigmine (0.04-0.08 mg /kg): 0.04-0.08 x 70 kg = 2,8-5.6 mg
• atropine (0.01-0.02 mg /kg): 0.01-0.02 x 70 kg = 0,7-1, 4 mg
Fluid
Preoperative: fluid for 8 complement fasting hours
2 cc x 8 hours x 70 kg = 1120 cc
intra-operative
Bleeding: ±20 cc
EBV: 75cc x 79 kg = 5250 cc
 EBL: 20 /5250x 100% = 0.38%
Bleeding <20% give crystalloid with ratio 3: 1  3xblood loss = 3x20 cc = 60

Maintenance fluid during operation:2-4cc / kg / hours = 2 x 70 kg = 140 cc
 Total fluid intake intraoperative = 1120 + 60 + 140 =
- 3B KAEN 500 cc  20 TPM
- Fasting until full awareness
- Giving O2 masks 3 lpm to SpO2> 95%
- Injection of Paracetmaol 1 g / 6 hours
