What Is Radiofrequency (RF)?
• Radiofrequency, or “RF”, is the term for
energy that is comprised of a very large
portion of the “electromagnetic
radiation spectrum” (EMS)
• Oscillating waves in the EMS with a
frequency (or rate of oscillation) from as
low as 3 Hz to the extreme of 300 GHz
and wavelengths as long as 100 Mm
(megameters) to as short as 1 mm are
considered “RF” energy.

Radio Frequency Principles
• The controlled use of RF produces
a highly efficient thermal effect on
tissue called for selective
electrothermolysis
www.sybaritic.com
Distribution of RF Energy
Depends on the geometry of the electrodes:
1. Mono-polar system (a single electrode)
2. Bi-polar system (two electrodes)
3. TRI-Polar system Focused Alternating Depth (three
electrodes)
The main difference between the modes is the way the RF energy is
controlled and directed to the target
www.sybaritic.com
Mono-polar System
• Delivers energy through one
small electrode (while a large
ground electrode is applied to
the body at a distant point)
• Electrical energy decreases
with distance from the
electrode
• Generally unpredictable due to
RF energy having to pass
through the entire body to
grounding electrode to
complete the circuit
www.sybaritic.com
TRI-Polar System
• Passes the RF current
between three electrodes
at a fixed distance apart TRI-Polar Electrodes
(A, B, C) A-C
• Software controls the
alternating current
between A-B,&B-C, A-C.
• A-B deep RF penetration, A B A
B-C less deep RF B-C C
penetration, A-C deep RF
penetration
A-B
www.sybaritic.com
www.sybaritic.com
Bi-Polar System
• Passes the RF current
between two electrodes at
a fixed distance apart
• Tissue resistance to bi-
polar RF energy creates
precise and controlled
local dermal heating
www.sybaritic.com
TRI-Polar/ Bi-Polar vs Mono-
polar
• The main advantage of TRI-Polar/ Bi-Polar vs. Monopolar is the
controlled distribution of RF energy inside the tissue

Skin Tightening:
Mechanism of Action
• Collagen Denaturation
• Collagen Cross-Link Breakdown
• Collagen Remodeling
• Fibroblast Stimulation
www.sybaritic.com
Collagen Types
• Collagen occurs in many places throughout the human body.
There are 28 different types of collagen described in the
scientific literature.
• Type I
– Most abundant type of collagen found in the body (comprising
over 90% of our total collagen).
• Type VII
– Forms the anchoring fibrils found in the dermal-epidermal junction
(DEJ).
• Types V & VI
– Most interstitial tissue associated with type I
www.sybaritic.com
Collagen 1
• Most abundant protein in
mammals (30% of the total
protein the human body).
• Responsible for the strength and
elasticity of our skin.
• Gradual degradation of collagen
leads to the characteristic lines,
wrinkles and folds that
accompany the aging process.
www.sybaritic.com
Collagen Fibers
• Tough bundles of collagen called
collagen fibers are a major
component of the extra-cellular matrix
that supports most tissue structure.
www.sybaritic.com
Collagen Cross-Links 1
• Collagen triple helix coil structure is
maintained by hydrogen bonds.
www.sybaritic.com
www.sybaritic.com
Skin Tightening: The Process
• First process of skin tightening
– both immediate de-naturation of collagen
fibers (due to contraction and shrinkage)
• De-naturation describes the major change to the tissues’
protein structures when subjected to stressors (such as
heat produced by RF conduction).
– subsequent long term remodeling
– formation of collagen by cross-linking
– new collagen production.
www.sybaritic.com
Skin Tightening: The
• Collagen fibers areProcess
heated with RF energy
– some of the cross-links are broken,
– causing the triple helix structure to
unwind.
www.sybaritic.com
Skin Tightening: The Process
• The safe temperature range to shrink collagen
fibers without destroying them is narrow (60 –
62C)
• When the collagen molecule is subjected to
temperatures between 60 -62C, the collagen
cross-links rupture and cause the immediate
contraction of the fibers to about 1/3 their
original length.
• The molecules, however, remain intact—
provided that temperatures do not exceed 75 C.
www.sybaritic.com
Skin Tightening: The Process
• Second process of skin tightening
– collagen remodeling, (10-12 ) weeks
• thermal damage in the dermis induces an
inflammatory response leading to the release of
extra-cellular precursors that stimulate fibroblast
activity and remodeling of dermal collagen.
• New collagen is rebuild in the areas where
it was lost due to the aging process
• Gradual proliferation of fibroblasts leading
to new collagen deposits in the collagen
depleted areas.
– reduction of tissue laxity
– improved elasticity
– reduction of lines and wrinkles.

TRIO XT Clinical
Expectations
www.sybaritic.com
Setting Proper Patient
Expectations 1
As with any other non-ablative aesthetic
procedure, the key to having satisfied
patients is to properly set their
expectations prior to the treatment
www.sybaritic.com
Setting Expectations 2
• First, patients should be advised that a
series of treatments are always required
for best results
• Each individual will respond differently to
RF treatments as a myriad of variables will
affect clinical results
• Collagen remodeling takes an average of
six months for completion of the process
www.sybaritic.com
Contraindications to
Treatment
• Pregnancy
• Abdomenal application (during menstrual period)
• Audiphones, cardiac pacemaker, etc. or other metal medical apparatus or
metal/plastic implant in the body (if you have a question, consult a
physician)
• Implanted intrauterine device (IUD)
• Gold wire implant
• The patient wearing a watch, necklace, or other metal jewelry.
• Physical skin disease or inflammation of the skin.
• Do not use directly on upper eyelid, throat (Adam’s apple or thyroid glands),
behind the knee or on overly sensitive areas.
www.sybaritic.com
Pre-Treatment
Considerations
• Patient must read, understand and sign appropriate
Informed Consent (invite to ask any questions and
provide answers to patient’s satisfaction)
• Cleanse face with soapy water (do NOT apply
moisturizer after cleansing) and dry thoroughly
• Take “before” photos (5 views plus areas of opportunity,
following clinic photography protocol)
• Ensure and reinforce appropriate expectations are set
(treatment experience, moderate improvement of
wrinkles and fine lines, timing of results, etc.)
www.sybaritic.com
Pre-Treatment
Considerations
• Ask if patient has a pacemaker or ICD (internal
defibrillator), and confirm whether patient is, or could be
pregnant. If yes, discontinue procedure.
• Ask patient about specific allergies to medications.
• Ask patient to remove all jewelry, shirt or blouse, and put
on gown.
• Apply alcohol to thoroughly cleanse the face and neck.
www.sybaritic.com
Pre-Treatment
Considerations
• If applying topical anesthesia, apply a minimum of 45
minutes prior to procedure start time:
Time administered: _____________
Give meds __ mg
Give meds __ mg
Give meds __ mg
• Prior to TRIO procedure, if topical was applied, remove
topical completely and cleanse area with acetone. Then
apply treatment “grid” markings (for face and neck use 4
x 4 cm squares; for the body use 8 x 8 cm squares) with
white eyebrow pencil.

TRIO XT Pre-Treatment
Steps
• 1. Select hand piece in accordance with the area you are treating:
– a. Mini Hand piece: For use around eyes, nose and mouth;
– b. Facial Hand piece (with integrated contact cooling): For use
on face, neck, arms and hands;
– c. Body Hand piece: For use on the body.
• 2. Apply a generous coating of conductive gel to area(s) to be
treated.
• 3. Depress foot switch to activate RF energy. Release the foot
switch to pause or halt the emission of RF energy.
www.sybaritic.com
TRIO XT Pre-Treatment
Steps
• 4. Always keep face of hand piece flat on skin with both electrodes
in contact at all times (apply very slight pressure) and keep moving
hand piece slowly back and forth or in small circular motions in
treatment area (4 x 4 cm or 8 x 8 cm) until target temperature (40
degrees C.) is reached.
• 5. Be careful not to lift the hand piece off the skin until you have first
released the foot switch. This will be more comfortable for the
patient.
• 6. The STANDBY key must always be pressed before you change
hand pieces, energy levels, cooling selection (if applicable), or time
selection. Press READY key to resume.
www.sybaritic.com
TRIO XT Treatment
Procedure
• 1. Begin each treatment at the 50% energy setting and apply current to
first section (either 4 x 4 cm or 8 x 8 cm area) for approximately one
minute. At the one minute point, check skin temperature with supplied IR
thermometer. The target endpoint for each treated section is to
comfortably reach a target surface skin temperature of 40° Celsius.
• 2. Throughout the procedure, periodically ask patient about her comfort
level and adjust energy setting accordingly.
• 3. Avoid stopping movement of the hand piece while the foot switch is
depressed and energy is active as this may cause rapid tissue heating and
sudden discomfort.
• 4. If the skin temperature is below 40° C. in the section being treated,
increase energy level in increments of 10% and continue as shown in steps
1 – 3 above. Go as high as 100% energy if necessary to achieve the
desired 40 degrees C. endpoint as long as the patient can tolerate the heat.
www.sybaritic.com
TRIO XT Treatment
Procedure
• 5. Once the section being treated reaches 40°C, this temperature
must be maintained for at least one (1) minute. In order to do
this without causing discomfort to the patient, it is helpful follow
these steps:
– a. Move to an immediately adjacent area and apply the hand
piece for 10- 15 seconds;
– b. Move back to the section that you are maintaining at 40°C,
and continue working the area until the clinical end point has
been maintained for at least a full minute. Continue in this
manner until the entire desired facial or body area has
been treated.
www.sybaritic.com
TRIO XT Treatment
Procedure
• 6. Sensitivity levels around the eyes may make the
treatment more uncomfortable and should be adjusted to
provide lower energy settings than other areas of the
face.
• 7. Note: The “average” face will require treatment of
approximately 16 4 x 4 cm “treatment zones”. If you
spend approximately one minute on each area to reach
the target end point temperature and maintain for one
additional minute, the average face should take from 30
to 40 minutes to treat with a single pass. Multiple passes
are not recommended.
www.sybaritic.com
Post-Treatment
Recommendations
• Post procedure: Cleanse face with gentle cleanser to
remove any remaining conductive gel and grid markings.
Follow with application of moisturizer and sunscreen.
• Be sure to take post treatment photographs (using the
same 5 view as pre photos), and schedule I month, 3
months & 6 months follow up photo sessions and
evaluations.
• Post-operative care instructions: Moisturizer with
sunscreen as aftercare and may use cold compresses
(i.e., ice packs) if experiencing discomfort.
