EVALUATION & INTERVIEW
• To evaluate my interventions I would give a one-year timeline and re-evaluate the mortality rates to see if they decreased
from 2019 to 2020.

• I would check hospital admissions related to heart disease in that year to see if patients were able to provide self care and
keep themselves from having any acute symptoms.

• I would check new diagnosis of chronic diseases from 2019 vs 2020.

• I would interview healthcare workers at Ft. Hamilton ER to see if they have seen a decrease in the amount of people coming
to the ER for acute issues related to heart disease. I would also go back to community and interview residents to see if they
have adopted any of the interventions and how they are doing with them.
