EMR is design to keep tract of patient’s full name, complete updated address, gender,
date of birth, insurance details, next of kin to notify in case of emergency, past and current
medications history, immunization history, known allergies, radiological and laboratory results;
medical and surgical history including past hospitalization.
It also includes documentation of the current admission on patient’s progress assessment, vital signs,
plan of care by physicians or other health care related profession and education given to patient that
can be retrieved from multiple location within the same hospital but governs by security ensuring
patient privacy and confidentiality (Sh. M. Syed Mohamed And Mohd. H, 2005)
