 Reason:

*The dried palms of the coconut

tree might fall unexpectedly, so
there’s a chances that someone
will get hit by the falling coconut
palm

*The school staff must command

some personel to cut the dried
coconut palm to avoid falling
Broken Ceiling
REASON:

This broken ceiling is a hazard to the

students especially this broken ceiling
is located along the way to the
classroom, the debris or part of the
ceiling may fall because of its

MITAGATION:

This should be repair and

make sure that it will not
break anymore
Construction Area
Reason:
There’a chance that accidentally there
will be some debris that might fall
along the path way of the students, it
might hit them unexpectedly

Mitagation:
The foundation of this things should be
strongly secure and there should be a
reminder or warning that tells the student to
be alert if some cases happend
Wet Floor at Comfort Room
Reason:
This is hazard to the students because
the floor is slippery that causes the
student to slip

Mitagation:
To prevent unexpected accident, the floor
must be keep on dry always and there should
be a note or reminder for the student to avoid
spalishing water on the floor it not necessary
Open Outlet
Reason:

This is hazardous to the students because we don’t

that there might be someone who will touch the outlet
especially if there is no warning sign so there’s a
chance that she/he might get like electrocuted if the
outlet has still a flow of electricity
Mitagation:
There should be a sign or a warning that
no one should use the outlet if its not
being told to so that it can be avoided
