What is your medical diagnosis? How long have you
been diagnosed with such condition?

 Patient verbalizes that she has been diagnosed

last March 22, 2017 until now and her
creatinine level regularly changes from
very high to very low and has edema
 4.4 creatinine levels; recommended for a low-

sodium diet
 What were your experiences before you
finally decided to consult and seek
professional help?

 Patient verbalizes that she experienced

pain, cramps, and numbness felt along
her lower extremities, lower back pains,
frequent urination, difficulty in sleeping
 How do you compare the physical manifestations
you had before your consultation and at present?

 Patient verbalizes that she often felt

sometimes- lower back pains, frequent
urination, difficulty in sleeping; lessened
degree of manifestations
 What is your current treatment
plan/regiment?

 Patient verbalizes that she eats 3x a day and

eats 1 cup of rice every meal, drink at least 5-8
glasses of water a day, low-sodium diet, eats
vegetables and meat, and follow the
prescribed medications
What is your perception of your illness?

 The patient verbalized that she was shocked

because of many different perceptions about health
conditions like for example in edema, if the edema
will deflate it can cause immediate death. However,
she also said that she was always thinking of her
health condition because she's worried what will
happen to her and to her children.
 How do you go about your daily activities?

 She already used to have such daily routines;

adheres to timely intake of daily medications
 She regularly walk every morning
 Patient B verbalizes that she can sleep up to 8
hours. She usually sleeps around 9:00pm and
wakes up to 5:00pm
 She verbalized that she had no difficulty of
sleeping
 Patient B does not take any medication to aid sleep
MEDICATIONS

DRUG NAME: CALVIN PLUS
ROUTE: Oral
DOSAGE AND ADMINISTRATION:1-2 tablets once a day.
INDICATIONS: Calvin Plus is used as a dietary supplement.
CONTRAINDICATIONS:
Calvin is contraindicated in patients with:
 conditions associated with hypercalcemia, hypercalciuria
 Chronic renal impairment
 Renal stones or history of renal stones
 Renal osteodystrophy with hyperphosphatemia
 Wilson’s disease; hepatic and biliary disease (copper)

Generic name: diphenhydramine hydrochloride
Brand name : Benadryl
Drug classification: Antihistamine, second
generation, ethanolamine
Dosage & Route: 25–50 mg q 4–8 hr PO.
Indications: Treatment of hypersensitivity reactions,
Treatment of motion sickness, Treatment of
Parkinsonism, Nighttime sleep aid, Antitussive
Contraindication:
 Use with other products containing diphenhydramine

Generic name: Pantoprazole Sodium

Drug classification: proton pump inhibitors
Dosage & Route: 40mg capsule taken upon increase of
creatinine level
Indications: Short-term treatment of erosive
esophagitis associated with gastroesophageal reflux
disease (GERD).
Contraindication: Hypersensitivity to pantoprazole or
other proton pump inhibitors (PPIs); severe hepatic
insufficiency, cirrhosis.

Drug classification: Corticosteroid, Glucocorticoid,
Hormone, Pregnancy Category C 
Dosage & Route 4 mg tablet 1x a every other day
Indications: used to treat conditions such as arthritis, blood disorders, severe
allergic reactions, certain cancers, eye conditions, skin/kidney/intestinal/lung
diseases, and immune system disorders.
Contraindication:
Serious infections except septic shock or tuberculous meningitis; viral, fungal
and tubercular skin lesions; admin of live virus vaccines. Preparations
containing benzyl alcohol preservative are contraindicated in infants.

Generic name: Losartan Potassium
Brand name : Natrazol
Drug classification: Angiotensin Receptor Blocker
Dosage & Route: 50mg tablet 1x a day, daily after breakfast
Indications: Losartan is used to treat high blood pressure
(hypertension) and to help protect the kidneys from damage due
to diabetes. It is also used to lower the risk of strokes in patients
with high blood pressure and an enlarged heart.
Contraindication:
 In patients who are hypersensitive to any component of this

product.
 Use cautiously with hepatic or renal dysfunction,

hypovolemia

Dosage & Route:1 to 2 capsules a day with or after meals
Indications: Prevention and treatment of iron deficiency
anemias, dietary supplement for iron.

Contraindication:  Hypersensitivity, Severe hypotension.
