Introduction
Accurate and thorough documentation is the backbone
of a sound approach to risk management; it provides
essential patient information, historical details about
the course of patient care, and a record of services
provided.
Electronic health records (EHRs) have not diminished
the importance of documentation, but they have
fundamentally changed the process of documenting
patient care, resulting in new documentation risks.
To address these challenges, healthcare organizations
need policies and strategies that reinforce EHR best
practices.
Objective of the study & sample
General Objective
 To assesse the data and the user usage performance of
Electronic Patient Records (EPR) or systems alike in
government hospital at Selangor and the policy
implemented.
Specific Objective
To understand the knowledge of user about EPR policy
and guidance of usage.
 To understand the user`s do and don’t in EPR system
according to hospital policy.
Does the hospital educated the user periodically about
EPR and when new technology is implemented .
Sample Size
16 or (12%) of IT hospital at Malaysia, Random staff in
multiple ward
Study Criteria & Duration
Study population
There are approx. 16 hospital, all random staff at ward
Inclusion Criteria
All staff working with patient example Specialist,
Doctors, Jr Doctor, Head Of Ward, Sr Nurse, Jr Nurse,
Dietician and Pharmacist
Exclusion Criteria
All government hospital that doesn’t have a EHR system
Staff with no direct contact with patient data, example
Patient Attendan
