APPLICATION GUIDELINES (For Membership
in the Cacao for LIFE Program)
 I – QUALIFICATIONS
A. Level 1 – Applicants are organized and registered
groups:
1. Agri-based cooperatives registered with
CDA
2. Barangay-based associations registered
with SEC and DOLE
3. Agri-based NG0s, POs and Corporations
registered with SEC
 B. Level 2 – Applicants are
bona fide members of approved
organized and registered
group’s applicant
II– REQUIREMENTS
A. For level 1 – Applicants shall submit
to PCO-BIDA Program Two (2) Sets of
requirements:

a. ACCREDITATION REUIREMENTS

b. LOAN REQUIREMENTS
a. Accreditation Requirements :
1. Certificate of Registration – Xerox
Copy
2. Master List of Members – Please use
prescribed format
3. List of Officers and Officer’s Bio-Data
(Profile) – Please use prescribed form
and attach also photocopy of Individual
Community Tax Certificate (SEDULA)
4. Organization’s Latest Financial
Statements of at least 2consecutive
immediate years, if existing for 3 years or
more
5. Board Resolution requesting for
accreditation and possible acceptance to
the CLP
6. Photocopy of land title or any other
tenurial instrument or authority of the
member over the lot to be covered by CLP
b. Loan Requirements
1. Board Resolution authorizing its
organization to apply for loan and authorizing
its representative to sign loan documents and
its attachment
2. Loan application duly signed by authorized
representative
3. Cacao Production and Purchase Agreement
(CPPA) duly signed by both parties
 4. Loan Contract between OWI
(or 1WIAVI and participating
organization duly signed by both
parties
5. Barangay Clearance of the
participating organization
B. For Level 2 – Applicants shall
submit to the Office of the
participating organization
(Coop or Association, etc.) with
the support and assistance of
PCO – BIDA Program, the
following requirements:
a. Pre-Loan Requirements:
1. Member’s Bio-Data with attached ID
pictures and photocopy of Community
Tax Certificate (SEDULA)
2. Photocopy of land title or tax
declaration or any other tenurial
instrument or authority over the lot to
be included in the project
 3. Barangay Clearance of
the applying member
4. Favorable endorsement
for inclusion in the project by
the organization’s Chairman or
President.
b. Loan Requirements:

1. Duly filled-up Loan Application

form with co-maker who is an officer
of the participating organization
2. Duly signed Participation
Agreement
3. Certificate of Attendance in
the Pre-Loan Application
Orientation-Seminar
4. Simplified Loan Agreement
between the participating
organization and its interested
and qualified member, duly
signed by both parties
