Monitoring and Evaluation Committee
 The committee shall be composed of three (3) members: Chair and two
members. For schools with less than three (3) teachers, clustering with
nearby school is highly encouraged. While schools with more than Ten (10)
teachers may have more than 5 members to this committee.
The School M&E shall
1. Assists the school head in the appraisal of the SBM level of practice;
2. Assists in the conduct of quarterly School Monitoring, Evaluation &
Adjustment (SMEA) based from your AIP;
3. Conduct monitoring and evaluation (M&E) on the performance of the
school as indicated in the Annual Implementation Plan (AIP);
4. Monitor and Evaluate the implementation of DepEd / SDO mandated
Programs, Activities and Projects (PAPs) in the school
School M&E will submit report to the SDO M&E the following:
 Quarterly Report on the performance of the school on the
Annual Implementation Plan (AIP) thru the conduct of School
Monitoring, Evaluation and Adjustment (SMEA)
 Report on the implementation of Programs, Activities &
Projects mandated by DepEd & SDO:
 Feeding, SLAC Sessions, DRRM drills, and etc.)
 MOOE Utilization
 Customers feedback satisfaction
 SBM Level of practice
 INSET & Capacity Building
 Income generating activities
 M&E Controlled Tools
