THE PHYSICAL ASSESSMENT IS THE STEP IN THE
NURSING PROCESS. IT PROVIDES THE FOUNDATION OF
THE NURSING CARE PLAN IN WHICH OBSERVATIONS
PLAY AN INTEGRAL PART IN THE ASSESSMENT,
INTERVENTION, AND EVALUATION PHASES.

is the striking of the body surface
with short, sharp strikes in order to
produce palpable vibration and Is the listening to sounds
characteristic sounds. produced inside the body.
The Physical Assessment was conducted to
determine the progress of the health status of
the patient. We conducted the physical
assessment last October 6, 2021 through an
online interview using an online platform called
Google Meet. We asked various questions in
order for us to determine her overall state of
health and for us to create a client-centered
Nursing Care Plan.
