INTRODUCTION

In today’s modern age where computer has become a way

of life, it is evident that a majority of the country’s
institution still do not adapt the high technology.
Particularly in most medical clinic facilities, daily clinic
transactions are still done on paper and the retrieval of the
necessary records are manual.
We all know the current state of the country because of
the pandemic, medical clinics are operating at great pace
and striving to serve as many patients at possible with the
best of their abilities. But as the years passed by, the
number of patients has grown and various medical cases
arise that the manual method of managing patients’
records, prescriptions, billing and appointment schedule is
no longer practical.
 The main reason why Sta. Filomena Medical Clinic
should utilize the web application of data record and
clinic management system is because it’s much easier
to record and manage the clinic’s schedule, patient
informationprescriptions and billing with the use of
the proposed system.
 Today the clinic is using the traditional method of
managing system, manually recording the information
on the paper and storing the records on the big
cabinets, same as retrieving the old files, by manually
finding the patients records on each shelf. This causes
extra work effort and time to the nurse, secretary and
doctors.
 Conceptual Framework
 The patient will make an appointment by sending the
necessary information to the clinic, the secretary will
schedule the clinic visit of the patient through
mobile/table or computer.
 The Initial records will be saved in the database. The
doctor and secretary nurse has access to the data
record and data record cannot be disclose to anyone
without the request or approval of the patient. The
secretary-nurse will manage the list of appointment
and will receive application notification when the time
is scheduled for a patient.
 The patient can request and receive consultation
record through email.
 Statement of Objectives

The main objective of the study is to

design and develop user-friendly, efficient
and organized Web Application of Data
Record and Clinic Management System for
Sta. Filomena Medical Clinic, this system
could benefit clinic to maximize the
management system.
Specially, the study aims to achieve the
following specific objectives:

 1.To design and develop a web-based application of

clinic data record and management system.

 Login Form for the Secretary-Nurses and Doctors.

 Appointment Scheduling.
 Notification for patient queuing.
 Retrieval of Patient’s consultation date, prescription
record and appointed Doctor.
 2.To establish database for the clinic data record
such as:
 Patient Information (Name, Age, Address, Contact
No. and Email-Address).

 Patient
consultation date, prescription record and
appointed Doctor.

 Appointment Schedules.

 Secretary-Nurse and Doctor’s ID number for

application access
