Deep Vein Thrombosis

Introduction

Deep vein thrombosis occurs when a blood clot

forms in one or more of the deep veins in your body,
usually in your legs. DVT can cause leg pain or
swelling, but also can occur with no symptoms.
Demographics and CC

This is a case of patient Z.O, a 60 year old male,

Roman Catholic, currently residing at General
Mariano Alvarez in Cavite. Came to the hospital
with a chief complaint of distressing pain in his
left leg.
History of Present Illness

2 weeks PTC, the patient became very busy in his

office and sat down for almost 8 hours. He only
stood up for a couple of minutes during bathroom
breaks. The patient felt pain on his left leg and took
up Acetaminophen (Tylenol) 500mg to ease the
pain.
History of Present Illness

An hour PTC, while at work, the patient felt a

distressing pain on his left leg and the patient
noticed a swelling in the painful area, hence the
consultation
Past Medical History

Patient is slightly obese. He has no known allergies

to medicine and food. Patient has no surgical history
.
Family Medical History

Patient’s father also had the same condition. All of

the family members' health are remarkable.
Personal and Social History

The patient is currently working as an accountant.

His diet is composed of mostly meat and fatty foo
d. He is a smoker (24 pack years).
Physical Examination

General: Patient is awake, conscious and coherent

HEENT: Head is normocephalic.
Vital Signs: BP= 130/80 mmHg, CR=88bpm,
RR= 20cpm T=370C
Inspection: There is redness in the affected area.
Palpation: calf is tender when palpated
Detailed Physical Examination

Homans’ Sign

The patient was instructed to extend his knee to

passively dorsiflex his right foot. Then the doctor
will squeeze the back of his leg and a pain in the
calf will indicate that the patient has DVT.
Diagnostic Examination

Ultrasound: The ultrasound shows a presence of

thrombus in the vein
Salient Features

• There is redness in the painful area

• Painful area is swelling
• Painful area is warmth to the touch
• The ultrasound has shown a presence of thrombus
• The patient is positive to Homans' Sign
Differential Diagnosis

DDX Rule in Rule out

Baker’s Cyst -Swelling - Ultrasound shows no

-Pain in the leg swelling in the
popliteal fossa

Lymphedema -Pain in the leg - Ultrasound doesn’t

-Swelling show a blockage in
the lymph nodes

Deep Vein Thrombosis -Pain in the leg

-Swelling
-There is a presence
of thrombus
Final Diagnosis

Patient Z.O. was diagnosed of DVT due to the

presence of thrombus shown in the ultrasound
and is positive to Homans’ Sign
Treatment and Management

Patient was advised to use compression stockings

to help cure the swelling and to take Rivaroxaban
twice a day for 3 weeks as blood thinner.

After a month, patient Z.O. doesn’t feel pain

anymore. He stopped drinking alcoholic drinks,
he started eating healthy, and does walking as
his daily exercise.
