1. Submission of ROV and documentary
requirements

2. Processing of Salary differential due to Salary
Adjustment

• Documentary requirements (NOSA):


 Soft and hard copy of payroll
 Approved working papers
 Approved NOSA
 Approved certification of leave with or without pay
 Approved service records
3. Processing and Computation of Individual Salary Claim

• First Salary Claim Computation:


 Working days of the month as divisor
 Deductions:
 GSIS (PS): 9% regardless of accrued number of days(Calendar days)
 PHIC: 1.5% of the basic salary 15 working days or more
 HDMF: P100.00 if 15 days or more
 Tax: depends on tax rate
• Succeeding Salary Claim Computation:
 22 days as divisor
3. Processing and Computation of Individual Salary Claim

• Substitute
 Working days of the month as divisor
 Deductions:
 GSIS (PS): 9% of actual days served (Calendar days)
• Maternity Claim
 30 days as divisor
 refer to school calendar on the exact date of vacation period (including Saturday and
Sunday)
4. Submission of SALN(c/o Neilmark P. Alesna)

• Hardcopy of transmittal received from ombudsman


• Softcopy of scanned SALN
5. Submission of Form 7 and DTR

• Concerns:
 Deadline of submission at Division level every 5th day of succeeding month
 Accrued leaves not posted/updated in leave management
 Time log in the DTR and IDLAR must the same
 There should be Time In and Out in the morning and afternoon
 With complete attachments
 Certification on quarantine period must be CALENDAR DAYS
6. Submission of Form 6 to be approved in the
Division Level (more than 15 days)

 Letter of Intent
 Form 6 (4 copies)
 Division Clearance (regardless of number of days)

• Sick Leave with or without pay (15 days or more)

7. Submission of Reinstatement Form and

• Data needed must be correct in the Reinstatement Form

 Duly filled-up reinstatement form (3 copies)
 Approved Form 6 (3 copies)
 Medical Form 41 (3 copies)
 Medical Form 211(3 copies)
 Approved Division Clearance
 If Maternity Leave (live birth) attach photocopy of birth certificate of the child
8. Application/Request to Travel Abroad

• Pursuant to Unnumbered Regional Memorandum dated April 29, 2019


re: Approval on Request for Travel Abroad
• 45 days before the departure submitted to Division Level
• Documentary requirements:
 Letter of Intent
 Form 6
 Division Clearance
 Endorsement from the School Head and District Head
 If more than 15 days submit PSU Form – Notice of Inactive Status
