    HOD
    OHS REP
    Persons familiar with equipment (its operation , maintenance and reason for locking out)
    Procedure
    Persons who placed the dander tag and lock must be contacted in the presence of signatories
    They confirm work performed and work to be done
    Details of work done should be given to the surpervisor
    Written procedures to ensure work has been checked must be provided
    Once inspection is done for fitness to return to service all signatories must physically inspect the
    equipment
    Cancel work permit
    End
