Medical (M), Family (F) & Social (S) history

 M: Serik Kanatov, 45 years old came with the


chest pain and shortness of breath. The patient
is obese and has diabetes type 2 (was detected
1 year ago), and he was prescribed to take
Rosiglitazone to improve sensibility to insulin.
No allergies to drugs or food.
 F: No information about family diseases
 S: He is married, has 2 children, work as

manager in oil & gas company. He smoked 20


cigarettes a day, but since detecting diabetes,
he smokes 8 cigarettes a day.
Physical & radiological exam

 Height/ weight: 180/110


 Blood pressure: 160/100
 Electrocardiogram (ECG): some slight
abnormalities
 Treadmill stress: the presence of blockages
detected, but need confirmation by angiography
 Angiography: due to diabetes and taking
Rosiglitazone, performing angiography was
refused
 Computer Tomography: coronary artery disease
was detected
Treatment
 Diagnosis: Cholesterol-containing plaques in
the right coronary artery

 Surgical treatment: percutaneous coronary


intervention (PCI) i.e. placing bare metal stent
 Patient did not eat or drink 6 hours before

operation and was given pain medicine


 No complications during the operation
Conclusion
 After the operation, blood flow through
coronary arteries improved
 Patient was prescribed special diet and

exercises
 Strongly recommended to stop smoking
 Medicines for lowering cholesterol level in the

blood were prescribed


 After advising with endocrinologist that was

observing diabetes in the patient, taking


Rosiglitazone was stopped.
