October 19, 2011 Outline Medical History Family & Social History Physical Exam Radiological Exam Conclusion/Diagnose & Treatment References Medical History Chief complaint: “I have a chest pain” Medical History: “Agatha McMaster, 40 years old, was brought to the Emergency department with a chest pain, shortness of breath (dyspnea) and fever.” Present illness: She had the swelling of feet and legs, dyspnea and chest from time to time during last 2 months. Past History: She did not have surgeries or any severe illnesses before. She does not take any specific medicines permanently. She has an allergy to tobacco smoke and alcohol. Family & Social History Family History: Her mother died from myocardial infarction at age 60, no information about father is available. Social History: She is married, has two sons of 20 & 18 years old. Agatha is a system administrator. She does not usually do physical exercises and work with computers about 12 hours per day. She does not smoke, does not drink, does not take drugs Physical Exam Vital Signs: blood pressure 140/90, temperature 38.70C Inspection: The patient is underweight, and have normal body symmetry. She has a pale skin color and looks exhausted Palpation: There is a swelling in legs and abdomen, also she has a distension of neck veins Auscultation: The heart sounds are irregular and mostly fast Radiological Exam Electrocardiogram (EKG): arrhythmia detected Echocardiogram (Echo): in the right atrium a tumor was detected Magnetic resonance imaging (MRI): the location of tumor was confirmed, the size of tumor 3.5 x 4.1 cm Conclusion Diagnose: Benign primary Angiosarcoma of the right atrium (noncancerous original cardiac tumor) Treatment: Surgical excision, no medications needed, but echocardiography was done several times per year, There are 2 years past since surgery had been done.
