BADAC Data Capture Form 3 – a two-page form used for capturing the
regularity and extent of various activities conducted by the BADACs,
aimed at raising the participation and awareness of the residents. These
include the conduct of Information, Education and Communication (IEC)
campaign activities, consultative meetings with stakeholders, and
activities in observance of the IDADAIT and DAPC Week.

BADAC Data Capture Form 5 – the purpose of this form is to monitor the
number of surrenderers who have been enrolled in and have completed the
CSAR or any equivalent reintegration program on a semestral basis. Similar to
BADAC Data Capture Form 4, data entry in this form restarts every semester
so that data in the preceding semester shall no longer be accounted in the
ensuing semester.
