Proforma
March to June 2022

AIM

 create a proforma that can be used to monitor what tests have been sent,
what results have returned, and which tests have not yet been performed
 display what treatments a patient has had for infection whilst on the unit
 display tests not performed which could prompt them to be done
Objectives

 Main Objective
 To assess and improve the documentation of microbiology results in the ITU
 Specific Objectives
 To assess number of patients with the proforma
 To assess the cultures done for each patient
 To assess the antimicrobial use for each patient
 To assess feedback on the use of the microbiology proforma
Method

 Cycle 1
 Introduce the microbiology proforma
 Sensitize the team on the proforma as part of the monthly big 3 agenda in the ITU
 Audit the proformas after 2 weeks
 Cycle 2
 Re-sensitise the team on the proforma,
 Audit the form after 2 weeks
 Get anonymous feedback on the form from the team
 Cycle 3
 Introduce the simpler form,
 Audit after 2 weeks
 Get anonymous feedback

Impression

 Good use of the proforma initially


 Less follow up in the subsequent days
 Similar compliance across the cycles in spite of simplification of the forms
 Improved perception that the form improves provision of care for the patients
Recommendation

 To add the proforma to the daily bloods form


 To continue sensitization on the importance of the form
 To include more of the team in filling out of the form, i.e the nurses
