various LDMs. This is to decide that we can
deliver and provide basic services to our clientele
services which are learner-centered.

M&E PLAN
1.7 – Monitoring and evaluation processes and
tools

ANNOTATIONS:

Thru LDM Implementation of M&E Plan, I was

able to identify the hazards or the risk level in the
implementation and monitoring. It made me
realized that an important source of learning in
real-time is application of M&E processes.
TA PLAN FOR TEACHERS
3.2 – Teaching standards and
pedagogies
3.5 – Learning Assessment
4.5 – Learning and Development of
School personnel

ANNOTATIONS:

In giving of TA to teachers, we should see it that

everything is aligned with the organization’s
vision and mission and is based on the
organization’s needs. To be more effective in
performing their function it is important that we
give them our TA.
1.In today’s time when it is not possible to go out and only study at home is
allowed, these changes are difficult for our learners. But even so, we must
continue our classes even if we do not keep up with the mode of learnings
today. Yes, it really reflects my abilities as a leader on how to handle this kind
of learning modality. Why? Because we learned to be patient, understanding,
and know how to extend help from parents who assist our learners at home.
2.In this LDM course, our school used the SLM (Self Learning Modules) REFLECTIVE SUMMARY
printed for distribution to our learners. In my point of view, the use of printed
modules has a great impact to learners, because they find it difficult to work on 4.2 – Professional Reflection and
that. They cannot talk to their teachers and no one to teach them, and some Learning
parents did not focus to their children.
3.But later, as the course goes on, I slowly understood the point that this course
can help us, school heads as well as the teachers, to improve in assessing
ourselves and our way of monitoring and assess the effectiveness of the
modalities in this time of pandemic. In conclusion, this LDM course also
opened a lot of opportunities to both the school heads and the teachers for they
are the key factors in making this delivery modalities successful. With the help
and guidance of some instruction coaches like our district supervisor are so
important to ensure the quality education we want to provide and deliver to our
learners. We must be strong, safe, healthy and help each other make this dream
come true. Mabuhay ang Edukalidad! Mabuhay ang Kabataan.

mitigation, and resiliency

ANNOTATIONS:

I considered this as a process of identifying the

risk description from the different components of
LDM Implementation with preventive action &
mitigating actions to be done.

ANNOTATIONS:

The sample learning assessment tool was the

who are provided with intervention activities.
MOVs ON LEARNING
ASSESSMENT
3.5 – Learning Assessment
ANNOTATIONS:

This sample Individual Learning Monitoring

Plan shows that learner was having difficulty in
his lessons and cannot submit his output on time
and gets a failing grade with the use of ILMP,
constructed by teacher, the learner was provided
with proper interventions, guidance and remedial
instructions by the learning facilitator. In this
way, it would be of great help for the learner to
answer all its task given and can now easily
understand & improve at areas they struggle
with.
MOVs SHOWING HOW SH

PARENTS, TEACHERS AND
STAKEHOLDERS

information, to exchange thoughts and learnings
and to help the teachers in doing all the task
related to LDM.

stakeholders as partners in all our activities in
school, they were called for an orientation about
the schedule of its distribution and retrieval of
modules weekly.
