 Committee Goals

 To improve patient safety in hospitals through


multidisciplinary collaboration in the development of
evidence-based and patient-centered transfusion
policies, participation in professional education, and
monitoring blood use.

 To provide an objective and accurate assessment of


.the use of blood and blood components in hospitals

 To make recommendations on all aspects of


transfusion medicine in order to promote the highest
standards for patient care.

 Committee Functions

 Ensure the optimal use of blood and blood products by


establishing guidance indications and triggers for the
transfusion of blood, blood components and blood
derivatives.

 Develop, approve and implement evidence-based


standardized and multidisciplinary policies, procedures and
guidelines that involve the ordering and administration of blood
and blood products in the hospital, including but not limited to:
– Blood components ordering and patient consent
– Handling of blood outside the laboratory.
– Use of blood warmers and infusion devices.

 Committee Functions

 Develop, approve and implement evidence-based


standardized and multidisciplinary policies, procedures and
guidelines that involve the ordering and administration of
blood and blood products in the hospital, including but not
limited to:
– Venous access.
– Premedication and addition of fluids and drugs other than
0.9%NaCL.
– Bedside Identification of the blood product and the
intended recipient.
– Management of transfusion adverse reactions.
– Monitoring of patient during and after blood
administration.

 Committee Functions

• Continuously monitor the following as recommended by the


AABB and CBAHI standards:
– Blood product ordering practice
– Patient identification for samples and administration of blood
components.
– Adequacy of transfusion services (blood availability and shortage)
– Blood components usage and wastage
– Appropriateness of use, through prospective and retrospective
audits.
– All adverse transfusion reactions and near misses
– Periodic review of infectious markers in donor population

• Review new products and services to determine


whether they should be made available at the
hospital.

• Review of need to make changes in the hospital-wide


policies or modification in the “controlled
documents/forms as deemed necessary.

• Develop and monitor continuous education of


hospital staff on transfusion issues.

• Develop and monitor quality KPI for different areas


of blood banking and transfusion medicine.

• Ensure the optimal utilization of therapeutic phlebotomy and


apheresis services in the hospital.

• Recommend actions for transfusion service improvement and


evaluates their effectiveness.
