PATIENT’S PROFILE:

 Age: 91years
 Current caloric intake: 600kcal
 Weight: 45kg
 BMI: 16kg/m2 (Underweight)
COMPLICATIONS:

Dementia
Low albumin level 25g/dl
Oedema
 Weight loss
SHORT TERM GOALS:

Kidney disorder management ( Nephrotic syndrome):


Introduce iron rich foods sources (dates,figs,apple,cherry)
Introduce vitamin C and D and iron supplements.
Low potassium and phoposours foods.
Low protein diet (0.5-0.6g)
Restriction of sodium(500-750mg) and
 Sugary foods
MANAGEMENT OF DIETARY POTASSIUM:

When kidneys working not properly potassium builds up in the body and cause great stress to
heart functioning.
Restricted foods such as banana, raisins.
 Good source of potassium foods such as apple, mixed vegetables.
MANAGEMENT OF PHOSPHORUS:

The high phosphorus level cause vitamin D deficiency.


It can effect calcium metabolizing hormones.
 Reduction of dietary phosphorus is done to prevent it’s accumulation in the
body.
MANAGEMENT OF CREATININE AND UREA:

 Lower creatinine levels by avoiding creatinine in your diet and eating more fiber and less
protein.
 Eating less red meat and fewer fish products may reduce the higher creatinine levels.
 Limiting the amount of protein and including more plant based foods may slow the rate of
kidney diseases.
MANAGEMENT OF ALBUMIN:

Soft protein sources like shredded meat,soup,broth.


 Vitamin D supplements
THIRST MANAGEMENT:

1000ml/day= 2 to 3 cups
Use small glasses, such as juice glasses, at meals
Limit the amount of salt in your diet
Keep a supply of hard candy, like mints.
Take sips, not gulps
 Sodium restriction
WEIGHT LOSS:

Give small portions of food


Nutritional smoothies can be given
Keep in mind the patient preferences
 Nutrient dense foods can be given.

Dementia Definition.

remember, think, or make decisions that interferes with doing everyday activities.”
MEDITERRANEAN DIET:
DIETARY MANAGEMENT OF DEMENTIA:

Mediterranean diet.
Fish, walnuts and almonds ( omega 3 fatty acids).
Turmeric ( curcumin).
 Dark chocolates, mushrooms containing zinc.
DIET PLAN:

Pre breakfast 1cup of green tea.


Breakfast Two white breads with fry egg
Snack Fruit and Vegetables salad(apple, Cherries,carrot , cucumber
Lunch Jau ka Daliya
Snack strawberries+ 1 cup of golden milk
 Dinner 1 Tortilla + Baked fish
