In General we found :

 75% People do not maintain proper health record

or information. No history of manual/physical
records is preserved.
It is also difficult to maintain all the health related
information's and make it available as and when
required.
Information's are not shared among medical
fraternity. (obviously with proper authentication)
 Due to non availability of proper records, Medical
fraternity has to reassess from the beginning, which
