COMPANY HISTORY/ DESCRIPTION

Queen M’s was established on January 26, 2005 by


Marjorie Religioso. It is to serve and cater some services.

Before, it only caters food and wedding but now it all cater
all kind of events such as wedding, meeting, birthdays etc.
Now, we are expanding our company by building other event hall
in other Provinces here in Quezon. Soon we will rise and open our
branch nationwide.

When: July 20, 2020 on Saturday at 10:00 A.M.
Why: To know who’s the best eater in Barangay.
How: 150 persons are fit in an Event Hall and zero cost
because its free viewing for people in Brgy. 3, Lucena City.

EVENT DESCRIPTION
This eating competition will show on July 20,
2020 Monday at 10:00 am at Barangay 3, Event Hall. This
eating competition is sponsored by Brgy. 3 Lucena City
and this event has 2 month preparations (including
audition and selection). This is a free viewing competition
to a public inside the Event Hall of Queen M’s.
THEME/CONCEPT
The theme/concept of the event will be “Who’s the
Beast Eater of them All?”. It will have a grand coronation
in the Event Hall. Audiences are residents of Brgy. 3 and
the guests are Brgy. Officials including SK’s and
Chairman. It is free viewing for them to entertain the
residents. There will be a 10 candidates for the
competition with cash prize.
Calendar preparation:

May 19, 2020 First day of planning for eating competition in Brgy. 3.
Creating a letters for request and conformation for venue, event
contact and other contacts.
May 30, 2020 Audition for those who wants to join.

June 10, 2020 Preparation for the event. Preparation for the motif and design
of the event.

June 21 - 28, 2020 Designation of tickets who wants to enter the event.

July 20, 2020 The day of the event of Hotdog Eating Contest.

This contract event venue made this 20 day of July, 2020, is by and between City of
Lucena City, whose business address is Cabana St. Brgy. 3 Lucena City and Erwin
Onato collectively referred to as the “parties”.
This contract regards the use of the property provided by venue, at the address
above, on 07/20/2020 which is does include services or equipment to plan, coordinate,
set up, deliver, perform/conduct, or tear-down/clean-up after the event. In
consideration of the mutual promises and convenants contained herein, the Parties
agree to follows:

that:
Hotdog Eating Contest will provide full professional
service including but not limited to event managing,
hosting, staging, directorship, and will provide the
necessary supplies.
TERMS AND CONDITIONS
1. Jommel Niere will pay the total amount of PHP 90,900 for the entire service provided.
One half of the total expenses PHP 22,725 is to be paid upon completion of this contract as
a deposit to hold the date, (note: deposit is not refundable) and the balance of the total
amount to be paid two (2) days before date of the event.
2. Real Event furnish labor and equipment, materials, including set up and breakdown.

PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK
In consideration of the acceptance and participation in the services of Hotdog Eating
Contest, their planners, owners, officers, volunteers, employees, team members and
all other persons or entities acting in any capacity on their behalf (hereinafter
collectively as Event Team, I hereby release, indemnify, hold harmless, and discharge
Hotdog Eating Contest, from any and all liability for injury or harm that I or anyone
might sustain and I hereby release, hold harmless and discharge Hotdog Eating
Contest, from any and all claims, demands, or causes of action, which are in any way
connected to my participation in this activity or use of equipment to the extent allowable
under Philippine laws.
1. l, Fernan Glindro, residence of Barangay 3, Lucena City,
acknowledges that my participation entails known and unanticipated risks
that could result in injury or damage to me, to property, or to third parties.
I understand that such risks simply cannot be eliminated without
jeopardizing the essential qualities of the activity. THE RISKS INCLUDE,
AMONG OTHER THINGS: FALLING OFF THE STAGE RESULTING IN
MUSCULOSKELETAL INJURIES INCLUDING HEAD, NECK, BACK,
HAND, AND ARM INJURIES. Furthermore, Real Event, employees have
difficult jobs to perform. They seek safety, but they are not infallible. They
might be unaware of a participant’s fitness or abilities. They may give
incomplete warnings or instructions, and the equipment being used might
malfunction.
2. I expressly agree and promise to accept and assume all the risks existing in this
activity. My participation in this activity is purely voluntary, and I elect to participate in
spite of the risks.
3. I hereby voluntarily release, forever discharge, and agree to indemnify and hold
harmless Fireworks Event Ideas, from any and ALL claims, demands, or causes of
action, which are in any way connected with my participation in this activity or my use
equipment or facilities, including any such claims which allege negligent acts or
emissions of.
4. Should Fireworks event Ideas, or anyone acting on their behalf, be required to
incur attorney’s fees and costs to enforce this agreement, I agree to indemnify and hold
them harmless for all such fees and costs.
5. I certify that I have adequate insurance to cover any injury or damage I may cause
or suffer while participating, or else I agree to bear the costs of such injury or damage
myself. I further certify that I am willing to assume the risk of any medical or physical
condition I may have.
6. I agree that if any portion of this agreement is found to be void or unenforceable,
the remaining portions shall remain in full force and effect.
By signing this agreement, I acknowledge that if anyone is hurt or property is
damaged during my participation in this activity, I may be found by a court of law to have
waived my right to maintain a lawsuit against Hotdog Eating Contest, on the basis of
any claim from which I have released them herein.
