Registration Form
1. Submissions should be received by March 1, 2013. 2. Submissions may be any form of writing: essay, poetry, short story, novella, etc. Please indicate type of submission on your registration form. 3. All submissions should be typed, double-spaced, and at least 12 font type. Students third grade and under may hand write instead of type. 4. Writing should be submitted in a report type folder with a clear front. 5. All submissions should be labeled with your name and age. 6. Category: Academic (You may submit two pieces of writing, or you may submit one piece of writing and participate in one additional academic category for the same registration fee. 7. Registration Due Date: February 15, 2013 8. Registration Fee: $10 per student ($15 per student after deadline) 9. $10 will enter student in one talent show category, two Academia competitions, or two Art or Photography entries. 10. Registration forms are available through co-op or online at www.classactacademy.weebly.com or www.classactperformers.com or request one by email at mwebster1998@gmail.com. Any registration form will do. If more information is required, I will email you with the requests. 11. Awards will be presented on Saturday, March 16, for all categories and events. If you are unable to attend Saturday evenings award ceremony, please make arrangements with someone else to get your rewards or include an additional $10 fee with registration to cover priority shipping for rewards. 12. Any adjustments or changes necessary for events will be made at discretion of coordinator. 13. Location of events: JSU/ McClellan Theater, 100 Gamecock Drive, Anniston, Alabama. 15. Sponsoring organization and facility arent responsible for missing items or accidents. By participating in event, you agree to terms above, any stated below and on registration form and other correspondence.
