MILLENNIUM EXCELLENCE AWARD PROGRAM

2008 ENTRANCE AWARDS

In 2008, the Canada Millennium Scholarship Foundation will grant 1,052 entrance awards to students beginning post-secondary studies for the first time as part of its Millennium Excellence Award Program. The entrance awards seek to recognize the leaders of today and tomorrow and emphasize the importance of supporting their contributions to our countrys future. Millennium excellence awards are not prizes for benchmarks achieved they are investments in the development of exceptional individuals who will have long, productive careers and whose accomplishments will benefit Canada as a whole. The Foundation distributes up to five per cent of its annual allocation through its Millennium Excellence Award Program. NUMBER AND VALUE The Foundation will grant the following entrance awards for use during the 2008-2009 academic year: 100 national awards of $20,000, disbursed in annual allotments. 241 provincial/territorial awards of $16,000, disbursed in annual allotments. 711 one-time local awards of $4,000. ELIGIBILITY CRITERIA In order to be eligible for an entrance award, a student must: Be a Canadian citizen, a permanent resident or a protected person within the meaning of the Immigration and Refugee Protection Act. Be entering his or her first year of full-time undergraduate studies leading to a first post-secondary degree, diploma or certificate. Be enrolling in a Canadian post-secondary institution (college, CEGEP , university or institute) recognized by his or her provincial student loan program in the 2008-2009 academic year. Be enrolling in a program of at least eight months duration leading to an undergraduate degree, certificate or diploma. Candidates for national and provincial/territorial awards must be enrolled in a program of at least two years duration. SELECTION CRITERIA Good citizenship, demonstrated by positive and verifiable service to the community Academic achievement Demonstrated capacity for leadership and ability to motivate others Demonstrated interest in innovation. RULES AND REGULATIONS Post-Secondary Admission: It is the responsibility of the individual to apply and gain admission to a college, CEGEP , university or institute in Canada. Entrance awards are conditional upon the Foundations receipt of notification of full-time enrolment from the selected post-secondary institution. Renewal Requirements: Provincial/territorial and national award recipients will be expected to maintain a high academic standing as well as demonstrate a continued commitment to leadership, community service and innovation. Application Instructions
YOU SHOULD READ ALL FOUR PAGES OF THESE APPLICATION INSTRUCTIONS THOROUGHLY. If you have any questions, please call the Foundation at 1-877-786-3999 or (514) 985-0026 or excellence@bm-ms.org .

STEP 1: GETTING STARTED Before you begin your application, please make sure you have read the cover page thoroughly. Check that you are eligible and that you meet the selection criteria. You may obtain more information by calling the Foundation or by consulting our Web site at www.excellenceaward.ca Check that you have all six pages of the 2008 application form and the one-page 2008 personal information form. STEP 2: COMPLETING THE APPLICATION FORM Additional application forms may be downloaded from our Web site at www.excellenceaward.ca. Use the original application form. Do not recreate or reformat the form on your computer; if you do, your application will not be accepted by the Foundation. Use a typewriter or a computer (minimum character size: 10 point). To use a computer to complete the application form, you may download the file from our Web site, complete it and print it. If you do not have access to either a typewriter or a computer, you may print neatly with blue or black ink only. Please do not use a pencil or other coloured ink. Applications that are illegible will be disqualified. Answer all the questions on the application form. Answer the questions within the space provided. Do not add appendices, extra pages or material such as certificates. They will not be read by the Foundation. Only applicants who are entering a post-secondary institution as mature students or have been out of school for longer than two years may include a one-page description of their employment and/or volunteer history. Have someone check your application for spelling mistakes. Sign and date page 1 of the application form. Use only the province of your permanent home address when you complete the Province/Territory of Residence section. If you are currently attending boarding school, your permanent home address is the address of your parent(s)/guardian(s). Include two copies* of the fully completed and signed six-page application form with the pages in the proper numerical order in the application package that you will send to the Foundation address that corresponds to your permanent address or to your preferred language. You will find these addresses listed in section STEP 10 of these instructions (page 3 or page 4).
* Please note that for all application documentation being requested in duplicate or two copies, you must submit either two photocopies or one original and one photocopy. It is suggested that you keep a copy of your application for yourself.

STEP 3: COMPLETING THE PERSONAL INFORMATION FORM The information contained in the personal information form will be used for all Foundation correspondence. Please ensure that the information that you provide us is complete, accurate and up-to-date. Type or print clearly in blue or black ink only. Please do not use a pencil or other coloured ink. Ensure you have filled in every field. Include two copies of the fully completed personal information form in the application package that you will send to the address on page 3 or 4 that corresponds to your permanent address or to your preferred language. STEP 4: THE ENDORSEMENT When choosing an endorser, make sure that you ask someone who knows you well. They should be aware of your involvements and be able to write of them in detail. Your endorser must complete and sign page 6. The endorser must submit, in duplicate, a signed one-page reference letter on letterhead. If the attached letter is longer than one page, the extra pages will not be read. Make sure you allow sufficient time for your endorser to fill in page 6 and to write the reference letter. Include two copies of the one-page reference letter in the application package that you will send to the address on page 3 or page 4 that corresponds to your permanent address or to your preferred language.

STEP 5: THE TRANSCRIPT/RECORD OF STUDIES Academic transcripts should provide your secondary school marks (grade nine/ten or secondaire III to date), results of upgrading courses and marks received on pre-university courses (if applicable). Transcripts and records of studies cannot be sent separately or at a later date. In Alberta, a copy of your latest report card bearing the seal of your school and the signature of a school official will be accepted. Include two copies of the official transcript of your academic record or record of studies in the application package that you will send to the address on page 3 or page 4 that corresponds to your permanent address or to your preferred language. STEP 6: FOR MATURE APPLICANTS ONLY If you will be entering a post-secondary institution as a mature student or if you have been out of school for longer than two years, please provide us with duplicate copies of the following documentation: A one-page description of your employment and/or volunteer history with a brief explanation of why you want to begin post-secondary studies. Please include your most recent record of studies. If you do not have a high school transcript or if you have completed your secondary studies more than five years ago, you do not have to include it. If you have been granted admission to your program on the basis of an admission assessment, include two copies of this assessment with your application. You should include results of upgrading courses and marks received on pre-university courses if applicable. STEP 7: PREPARING PACKAGE TO MAIL You may only submit one application package to one of the Foundations P .O. Boxes. Any subsequent application packages received by the Foundation will be discarded. All application material must be mailed in one 9 x 12 envelope to the Foundation. Do not fold your application. Include two copies of the fully completed PERSONAL INFORMATION FORM. Do not staple these together or to the application form. Sign and make a photocopy of your application. Staple the remaining pages of the APPLICATION FORM together in the following order: Pages 1 through 6 and one-page reference letter + transcript/record of studies/admission assessment + one-page description of employment and/or volunteer history if you are entering a post-secondary institution as a mature student or if you have been out of school for longer than two years. Submit both copies of your complete application package The complete application package must be mailed to the address on page 3 or page 4 that corresponds to your permanent address or to your preferred language. STEP 8: APPLICATIONS WILL BE CONSIDERED ONLY IF: Your application form is fully completed, legible and signed The endorsement page (page 6 of the application form) is completed and signed You have submitted in one envelope: O Two copies of the PERSONAL INFORMATION FORM O Two copies of the APPLICATION FORM O Two copies of the one-page reference letter (signed by endorser) O Two copies of the academic transcript or record of studies or admission assessment Your application is mailed to the proper P .O. Box and postmarked no later than December 14, 2007. STEP 9: MAILING DEADLINE POLICY Your application to the Foundation must be postmarked, (Canada Post) by December 14, 2007. You must use Canada Posts regular mail, Xpresspost or Priority Courier service because the Canada Post office boxes used by the Foundation do not accept hand deliveries or packages sent by privately operated couriers. Packages sent by privately operated couriers will be returned to you. The Foundation is not responsible for any postal errors and it will not accept any application that is postmarked after December 14, 2007, regardless of the circumstances. It is your responsibility to ensure that your application package is postmarked by the deadline date. Therefore you should take into account the time needed by the postal service for processing your application and allow sufficient postage when mailing it. If your application is incomplete, incorrectly addressed or late, you will be disqualified from the competition.

The Foundation is not responsible for applications that are lost before they are delivered to one of the following addresses. Applicants WHO HAVE NOT RECEIVED an acknowledgment of receipt must contact the Foundation before March 1st, 2008 in order for their application to be considered.
