PERFORMANCE APPRAISAL

LILIBETH JOCSON Office Staff (Probationary) Dear Ms. Jocson, We would like to inform you about the results of your performance appraisal conducted last month by your superior, Ms. Daisy Ng, in consonance with your probationary employment with our company. Your performance was deemed unsatisfactory. The following were her findings: o o o o Slow in completing office reports requested; Complains a lot about overtime work; Constant tardiness (at least twice a week); Four unexcused absences within the month.
