Lorenzo College Dean College of Information Technology Sir: Last December 10, 2012, I was admitted in Amai Pakpak Medical Center for the initial diagnosis of acute appendicitis. But later that night, the doctor diagnosed me with a ruptured appendicitis. Due to the complications it would give me, I was immediately scheduled for appendectomy or the surgical removal of the appendix the next day, December 11, 2012. The doctor has recommended me to rest for at least 3 weeks after the operation. In view of this happening, I am requesting your office to allow me to graduate in absentia this coming 48th Commencement Exercises on December 15, 2012.
