Action Exact mechanism unknown; appears to decrease polysynaptic responses and block posttetanic potentiation.
Indication Epilepsy, acute mania: maintenance of bipolar affective disorders. Alcohol withdrawal disorder.
Contraindication Pregnancy (Category D). Hypersensitivity. Severe hemic disorders. Severe bradycardia (less than 50 times/ min).
Adverse and Side Effect Mild, mostly transient and dose dependent. Dizziness, ataxia, allergic skin reactions, leukopenia, vomiting, accommodation disorders. Diplopia, liver enzyme elevations, SLE-like syndromes: peripheral neuritis, hallucinations, agitation, depression.
Nursing Considerations -Assess for seizures. -Monitor liver function tests and urine function test. -Caution patient to avoid activities that require alertness. -Advise patient to use sunscreen to prevent photosensitivity. -Advise patient not to discontinue medication quickly after long term use. -Teach patient to report immediately to physician if with following reactions: chills, rash, lightcolored stools, drak urine, yellowing of skin/eyes, abdominal pain, sore throat, mouth ulcers, bruising, blurred vision, dizziness.
