I am pleased to submit my proposal for the Structural Assessment & Design of the Existing Two Storey Residence with deck with proposed additional floor . Please find in the following the scope of work covered by this proposal: I. Scope of Work: 1. Structural Assessment and Analysis of Existing Structure ---------------( PHP22,000) The structural investigation and assessment of the existing structure will be based on the as-built plans, geotechnical report, and material test results that will be provided by associating consultants or engineers. This determination will tend to evaluate whether the structural condition of the existing building will be safe for any plan of extension or renovation. If the structure is identified unsafe, retrofitting design is recommended. 2. Preparation of Retrofitting Design of the Existing Structure -------------(PHP 30,000) (This service will only be provided if the building is indeed identified to be perilous for any building extension). This task will involve design and detailing of retrofitting of the structural elements of the existing building. The preparation of the drawings necessary for the construction of the work is also covered under this task. 3. Sign and Seal of Plans ------------------------------------------------------------(P7,000.00) (For any served documents) II. Submittals: 1. Assessment Report / Structural Analysis and Calculation 2. Structural retrofitting drawings (If applicable) In view of the above-mentioned professional services, I am proposing an amount of fifty nine thousand pesos only (P59,000.00).

III) TERMS OF PAYMENT 1. Down Payment - 30% = PhP 17,700.00 2. The remaining full balance will be paid upon the completion and submission of the assessment report or structural retrofitting drawings.
