English Language Learner / ELL Support Documentation for Student Files To be completed by District ELL teacher

Beginning of year 1. Referral form a. Sent home by school for students who are landed immigrants only b. Signed by parents giving permission for ELL support c. School send copy to Student Support Principal and copy to ELL district teacher 2. Woodcock Munoz testing report a. ELL teacher to prepare and send original to Student Support Principal b. Copy of report for ELL support teacher files c. Original Woodcock Munoz test in ELL support teacher files 3. Student writing sample- unedited a. Original or copy to Student Support Principal b. Copy for ELL support teacher files 4. Student report summarizing current student status and placement a. Written in consultation with classroom teacher, school ELL teacher or School Based Team (SBT) b. Original to Student Support Principal c. Copy for ELL support teacher files 5. Annual Instruction Plan (AIP) for student a. Created in consultation with classroom teacher, school ELL teacher or School Based Team (SBT) b. Original AIP to be sent to school principal for review c. School principal to sign, make a copy for classroom teacher d. School principal to forward original to Student Support Principal e. Copy of AIP for ELL support teacher files

Mid-Year Reports 1. Consult with classroom teachers regarding progress of student 2. AIP- update and revise as needed 3. Schools with their own ELL teachers will provide progress reports for their own students

End of Year
1. Woodcock Munoz summary report a. Original to Student Support Principal b. Copy for District ELL teachers file 2. AIP -review progress with teacher a. Copy for updated AIP for District ELL teachers file-should include handwritten notes and stickies on current AIP 3. Student report summarizing student progress throughout the year a. Written in consultation with classroom teacher, school ELL teacher or School Based Team (SBT) b. Copy to Student Support Principal c. Copy for District ELL teachers file 4. Report card comments a. Summary of progress based on AIP, WM testing and teacher consultation b. Copy for school file c. Copy for classroom teacher/report card 5. Documentation of student support
