This monitoring tool was developed to gather information on the readiness of all schools for the SY
2022 2023. The instrument focused on the school’s preparations for the management of human and physical
resources which will be used to provide Division data for more effective planning, intervention and policy
formulation. Likewise, this instrument proposed a contextualized assessment that takes into consideration the
policies issued from the Division, Region, and Central Offices in preparation for the opening of School Year
2022-2023.

coordinators based on emerging needs for the

gloves, clean water, soap and hand sanitizer or 70%
alcohol.

may be needed in times of emergencies.

families specifically the contact details.

shelters during disasters and emergencies.

C. School Health Protocol

1. The school implements safety health protocols


based on IATF on COVID 19.

and soap.

d. A school staff ensures temperature checking of


all individuals upon entering the school
premises.

distancing during transactions.
