MONITORING CHECKLIST FOR SCHOOL READINESS

1 - Not Ready - means that you have least or not at all implemented the program/activity.
2 - Less Ready - means that you have implemented 50% -75% of the program and expected activities.
3 - Ready - means that you have implemented 76 - 90% of the program and the expected activities.
4 - Most Ready - means that you have implemented 91% - 100% of the program and the expected activities.

A. SAFE OPERATIONS

1. Does the school establish safe entrance, exit, and contact tracing procedures for all concerns
(learners, teachers, parents/guardians, school personnel, etc.)?
2. Does the school have a proactive COVID-19 Task Force and local hotline/help desk that connects
and coordinates with the hospital and the LGU?
3. Does the school have an assigned/designated health safety officer/s who takes charge of the safety
protocols, procedures and mechanisms of the school? (Health safety officer may come from the
school stakeholders, i.e. PTA, Barangay Officials/personnel.)
4. Does the school have an adequate supply of face masks, face-shield, and protective gears as
replacement of those lost or damaged PPEs for teachers and other personnel?
5. Does the school provide a daily monitoring tool for school-goers?
6. Does the school set up a proper sanitation and hygiene facility (handwashing station, footbath, etc.)
for school-goers?
7. Are school's safety procedures, protocols, and mechanisms PWD-Friendly?
8. Does the school have a designated waiting area for parents/guardians during distribution and
retrieval of modules and self-learning activity sheets?

9. Does the school have a well-maintained and safe physical environment which includes but not
limited to low noise levels, cleanliness, access to clean air and water, and absence of overcrowding
and ensures the overall health and well-being of all school goers and personnel?

10. Does the school ensure conditions that reduce disease transmission, safeguard essential services
and supplies and promote healthy behavior? This includes access to soap and clean water for safe
handwashing, procedures when staff feel unwell, protocols on social distancing and good hygiene
practices.
