Location of Key Family Documents Every family has important documents that are time-consuming, difficult and, in some

cases, impossible to replace. The key to keeping up with them all is to know exactly where they're hiding in the event you, your family or someone you designate need to get to them either in the routine course of family business or in an emergency. Fill in this worksheet, print it out and keep it in a secure location where those who might need it can access it. Note: Letters in paranthesis, such as (sb), suggest where each document is best kept, though your situation may dictate something different. If, for instance, you need to keep your birth certificate in a lockbox at home, simply change the (sb) at the far right of the column to (lb) to reflect that. Also, just in case you need information quickly from a document stored in a bank's safe-deposit box, it's a good idea to keep a copy in your lockbox at home. (sb) = Safe-deposit box; (lb) = Fireprood lockbox or home-safe Documents Birth certificate(s) Death certificate(s) Marriage certificate(s) Divorce decree(s) Child custondy/adoption decree(s) Social Security card(s) Passport(s) Estate documents Will(s) Living Will(s) Trust(s) Power(s) of attorney Funeral arrangement documents Deed and titles to property Real estate Auto(s) Other Stock & bond certificates (not held at a brokerage) Insurance policies Home Auto(s) Life Umbrella Health Long-term care Location
