AFFIDAVIT OF LOSS I, FRANCIS XAVIER S. SINON, of legal age, single, residing at 655 Canelar Street, Zamboanga City, Philippines, after being sworn to in accordance with law, depose and say that: 1. I am a holder of a student identification card issued by the Xavier University College of Law-Zamboanga bearing the ID No. 110186; 2. On the afternoon of June 10, 2013, when I arrived in the Administration Office of the Xavier University College of Law-Zamboanga, I found that my wallet was no longer inside the backpack that I carried with me; 3. Contained in my wallet was the above described identification card and other pertinent Ids; 4. That diligent efforts have been made to search for such, but the ID was nowhere to be found; 5. That said School ID Card was not confiscated by the University College of LawZamboanga for violation of any school rule and regulation; 6. That I am executing this affidavit to attest to the foregoing facts and request for the replacement of the said school ID from the proper school authorities and for whatever legal purposes this may serve.
