AFFIDAVIT OF LOSS

That I, JOSEPH HOMER V. SASTRADO of legal age, Filipino,

single and a resident of Ulamian, Libungan, Cotabato, after having been
duly sworn to in accordance with law, do hereby depose and say:

That sometime on October 30, 2014, I was at our school campus at

Notre Dame of Midsayap, College, Midsayap, Cotabato;

That while at the school campus I lost my wallet which contains

important cards and documents;

That among the cards that were lost and documents are the
following:
1. Landbank ATM Card
2. Metro Bank ATM Card
3. One Network Bank ATM Card
4. School ID, (Notre Dame of Midsyap College)

That I exerted diligent efforts to look for and locate the

aforementioned card and documents but the same was not or located;

That the aforementioned cards and documents were not

confiscated by lawful authority or encumbered to any person or entity but
was lost;

That I hereby inform the authorities particularly the concerned

banks and school of the loss of my ATM Cards and ID and further request
for the issuance of another ATM Cards and ID;

That I execute this affidavit to attest to the truth of the foregoing

facts, and for such other legal intents and purposes the same may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 3 rd day of

November 2014, at Midsayap, Cotabato, Philippines.
