NURSING CARE PLAN

ASSESSMENT
Subjective: Kain naman siya ng kain pero yung timbang niya bumababa simula nung kumain siya ng barbeque na medyo hilaw ang luto ko. Lagi-lagi ko naman sya binibigyan ng vitamins pero parang walang epekto. As verbalize by clients mother. DIAGNOSIS
Nutritional Imbalance: Less than body requirements r/t parasitic infestation Definition: Intake of nutrients insufficient to meet metabolic needs.

PLANNING
STG: At the end of 1 week nursing intervention, client will be able to: Be absent from parasitic infection as evidenced by (-) stool examination. Increase body weight within normal range, 40-45lbs. LTG: At the end of 1 month nursing intervention client will be able to: Maintain normal body weight. Prevent acquiring of parasitic infection again.

INTERVENTION
Dependent: Administer antihelmentic as ordered. R: Antihelmentic drugs eliminates parasite on the hosts body, thereby helping to improve weight gain. Independent: Advise mother to give alternative treatment such as drinking 4ml of garlic oil or 6ml castor oil. R: Garlic oil and castor oil are believed to be an effective and affordable remedy for parasitic infestation; it is believed to help loosen the parasites attachment to the intestines and these parasites hinders bodys acquisition of nutrients thereby decreasing body weight. Advise mother to avoid intake of Provide health teaching about proper food handling techniques. R: Being knowledgeable about proper food handling techniques will prevent possible acquisition of pork tapeworm once again thereby preventing weight loss.

EVALUATION
STG: At the end of 1 week nursing intervention, goal was successfully met as evidenced by: (-) stool examination. Decreased abdominal girth of 25cm. Weight gain of 45 lbs from 25 lbs. LTG: At the end of 1 month nursing intervention, goal was successfully met as evidenced by Normal body weight of 48lbs. No acquisition of parasitic infestation once again.
