representative so that you can sign for the vehicle. This form will be retained until the vehicle
is handed back at the end of the loan period.

D = Dent, S = Scratch / Scuff, M = Missing, C = Chipped / Crack & H = Hole


All Faults found on the vehicle prior to and after the loan period are to be reported to the group
representative. You, as the driver have the responsibility to comply with Civil Law in relation to the
vehicle and the journey. If you have any questions ask the group transport representative. Upon
completion of your loan period the vehicle should be refuelled, washed down and cleaned out.
I certify that I have read and understood the above statement and have carried out a full check and am
happy that the vehicle I have taken over is in a clean and serviceable state and I have the correct
documentation for my journey.
