Dear respondent,

Good Day!

I am Clieza Mayol, a 4th year Fashion Design student in the University of San
Carlos and I am conducting a thesis paper entitled Assimilation of Cosmatesque
Technique Using Eggshells for a Bridal Collection as compliance of my class in
FD431: Workshop 1 and FD441: Workshop 2. This study aims to explore the
works of the Cosmati and be able to assimilate their technique known as
Cosmatesque using eggshells for a bridal collection. I would like to ask for a few
minutes of your time to answer this questionnaire and to answer it honestly as the
results of this will affect the outcome of this research paper.

Thank you very much.

Sincerely,
