AFFIDAVIT TO CHANGE UNIT

From HONDA MC: Plate # HV-9710 to Plate # IR4330

I, RELLA A. LABASBAS, of legal age, Filipino citizen, married

and a resident of B10, L9, King George St., Kassel Homes Subdivision,
Naga-Naga, Tacloban City, after being sworn according to law, depose
and state: THAT--1. I am the operator of a motorcab for hire (MCH) with Police
Number 0237 issued by the Tacloban City Permits and Licenses
Division, and run by a Honda TC unit with Plate No. HV-9710 and
covered by Certificate of Registration No. 74001982 dated 12-10-03 of
the LTO in my name.
2. However, I am now changing the old Honda TC unit with Plate
No. HV-9710 with a new Honda MC unit with Plate No. IR4330 covered
by Certificate of Registration No. 2303193-4 dated 3/13/2006 of the
LTO in my name.
3. This affidavit is executed to attest to the truth of the above
changing of Honda MC unit with Plate No. HV-9710 to Honda MC unit
with Plate No. IR4330 of the MCH with Police No. 0237, and for
whatever legal purpose this may serve.
