PRINTED PERMISSION TO VIDEOTAPE AND TRANSMIT A CONCERT

In Athens, today, July 2015,

I, the undersigned .(henceforth the Artist), declares and
acknowledges the following:
Based on the private contract on July 5th 2015, made with the company
Akti Voulas SA tourist, maritime and hotel enterprises, with headquarters
in Mochato, Attica, 95-97 Karaoli Dimitriou street, with VAT Registration
Number 800398034, Tax Office: Commercial Companies Piraeus Office, the
Artist has agreed to participate to the event/concert which the latter one
organizes on .. July 2015 in the coast with the distinctive title
SOUTH COAST ( PLAZ VOULA). This concert will be entitled ALL STAR
CONCERT and will be sponsored by the station RYTHMOS FM, while many
artists, musicians, singers etc. will participate.
Hereby the Artist accepts the videotape recording of extracts from the
event which will include her participation and performance, as well as
extracts from the preparation (making-off and back-stage) by the
communication sponsor. These extracts will be transmitted through the
Internet and the social network media, as well as the music platforms
exclusively for the purposes of promotion and advertising the event and
the participating Artists in this event. To this end the artist grants with the
present document, without financial recompense, permission to videotape
her performance and transmit extracts through the broadcasting media of
ANTENNA Group in Greece and abroad, for a time period of ninety (90)
days from the date of the event and exclusively for promotion purposes
(for example reportage and visualized reports to informative and
entertaining programs).
