1. Prior to commencement of operations, was it verified the fitness for duty of all personnel
involved in the unmooring of the vessel?

Is the Passage Plan for the intended voyage available on the Bridge and acknowledged by all
Deck Officers?
3. Are charts and nautical publications for the intended voyage corrected up to date, with
courses laid off and stowed in useable sequence?
4. Are the latest weather reports and navigational warnings available on the Bridge and the
Master informed on their relevant contents?

Was a log entry made by the Master showing the following data:
a. Anticipated navigational draught?
b. Controlling depth, including at the facility or anchorage?
c. Anticipated under-keel clearance estimated for the transit?
6.
Was the main engine confirmed ready by the Duty Engineer and the Master informed
accordingly?
7. Were the Emergency Generator and Emergency Lighting tested and performance/result of
tests recorded in the relevant logbooks?
8. Was the following equipment checked / tested ready for use?
h. Navigation and signal lights, including searchlights, signaling lamp, morse light
i. Sound signaling apparatus, including whistles, fog bell and gong system
j. Window wipers/clearview screen arrangements

Was a visual examination carried out to emergency towline(s), anchor releasing mechanisms,
and mooring lines prior to getting underway?

Were the Deck and Engine Room Pre-Sailing Check Lists satisfactorily completed, and the
results of the checks positively reported by the responsible officers?
16. Are cargo details and stability information available on the bridge?

.Any NO GO situations are to be noted in the logbook and communicated with shore based management in order to
establish an appropriate course of actions
