AFFIDAVIT OF PARENTAL CONSENT

I, RYAN M. BAUTISTA , of legal age, married, Filipino citizen, and
resident of Baliwasn Chico, Zamboanga City , under oath, do hereby
depose and state:
That I am the biological father of the minor child
MATHEA CASSANDRA, who was born on March 29, 2015
at Zamboanga City ;
That I am allowing my child to travel in Quebec with
her biological mother, MARY JOY BAUTISTA, who will be
moving in the said foreign country;
That I am hereby giving my full consent to MARY JOY
BAUITSTA and my child for their intent to be a permanent
immigrant at Quebec as long as their travelling documents
are made available;
That I am executing this affidavit to attest to the
veracity of the foregoing facts and for whatever legal
purposes this may serve best.
