Dear Essay Tutors:

This Essay Tutor Report form aims to help you report your interaction with each student. We hope that you can use the Essay Tutor Report to record your working details.

What you need to do is to fill the following three parts:

1. The essay topics on the second volume of the form:
2. Each date when you makes changes over each version of the essay
3. The NOTES part is for you to write something to remind you about the students or the essay
You are required to return this twice a month to me. You can return the form to our office on the 1th and 15th of each month, or 10th and 25th of each month---You decide when to
return this form.

Now, most of essay tutors have been assigned at least one student, and we know that you have done great during your first meetings with the students. Thank you so much, our
students say so many good things about you.
