Since I first began completing assessments at my internship, I
have learned what works for me in terms of how I prepare myself and perform my assessments with patients. I have also learned how to approach people with various diagnoses, such as congestive hear failure and the things to tell them. For example, when going on home visits with the CCTP coach, I have learned which tips to share with the patients in regards to their conditions.

They have also learned what types of specific questions and procedures work best with certain populations of people. Also, for example, when working with a patient who has been diagnosed with a health condition, such as congestive heart failure, I now know what types of questions to ask them and how to address their diagnoses with them.
