Practice Behavior #7.1


Utilize conceptual frameworks to guide the process of

assessment, intervention, and evaluation.

As a social work intern performing initial adult assessments, I


am constantly using these processes. We use assessment by
asking the questions needed for discharge planning. We then
use this gathered information to figure out what types of
interventions are needed. After putting these interventions
into action, we must then evaluate them for effectiveness to
ensure that the patient has received the best care possible.

Critique and apply knowledge to understand person and

environment.

For example, I went along with the CCTP social worker on a home
visit to the house of a patient who had recently been discharged
from the hospital with a diagnosis of CHF along with both Diabetes
and COPD. When we entered the house, it smelled strongly of
smoke and fried food. Because of the patients diagnoses, he is
supposed to watch his salt and sugar intake and stay away from all
smoke. He lives with his mother, who smokes in the home and
always cooks and buys foods that are unhealthy. He was clearly
non-compliant with the advice of his doctors. Adhering to these
instructions is made more difficult by the environment in which he
lives.
