Description of artifact and how the artifact meets the standard:
Progress reports, report cards, and data sheets are all great ways to provide multiple forms of evidence of student
learning and growth to students, families, and staff. This parent teacher conference note is used to show how evidence
from student learning is shared with families. This data conference report is presented and discussed with parents at
conferences.

Reflection on artifact (what went right, what went wrong, how to make better)
The data conference report is a great way to combine a students academic scores into one sheet. It is a good source of
evidence showing student learning because it summarizes the students assessment in all content areas. It shares with
parents their kids strengths and weakness, and also provides an opportunity to share different suggestions to use at
home. I also include percentages on this data conference report to help meet the need of my parents. Most parents find
it hard to understand our 3, 2, and 1 system. Using percentages helps them see their kids scores in a concrete way. I
would like to improve this data sheet by including the students math and science teacher. We rotate for these subjects
and it would be nice to have a name written down, and also what level they are in for math interventions. This will help
me present a true overall academic data sheet to each parent during conferences.
